Accountable care collaboration enhances how the health system cares for patients
Physicians will participate in UnitedHealthcare’s new NexusACO™ health plan in Southern California
Local employers among first in the country to offer pioneering option that expands access to quality care and lowers costs

CYPRESS, Calif. (Jan. 11, 2017) – HealthCare Partners, a DaVita Medical Group, and UnitedHealthcare are launching an accountable care organization (ACO) program to help improve people’s health and satisfaction with their health care experience.

HealthCare Partners’ physicians and specialists will provide care in Los Angeles and Orange counties under a value-based, patient-centric care model focused on keeping people healthy. As of Jan. 1, under the new ACO, HealthCare Partners is able to offer participating patients access to a personalized health team that includes a doctor and an ACO Unit, a dedicated staff of care managers and care coordinators who conduct health risk assessments, identify hospitalized ACO patients, coordinate transitions of care, and ensure patient care and health care resources are available. 

HealthCare Partners also extends medical management programs to patients such as disease management, specialized high-risk programs, home care and palliative care programs.

People enrolled in certain UnitedHealthcare health plans are eligible to benefit from this collaboration, including those at larger companies who have enrolled in UnitedHealthcare’s new national ACO product, NexusACO™. The product features a network of Tier 1 physicians who have proven results for delivering quality and efficient care. It is available in 15 markets across the country, including Southern California, to self-funded employers with 100 or more employees.

Many people who have gone to the doctor or hospital – particularly those with complex or chronic illnesses – have found they often have to connect information from each of their doctor’s visits themselves, and have sometimes received duplicative tests or care that isn’t coordinated.

Through the NexusACO plan, UnitedHealthcare and HealthCare Partners will work together to better coordinate patients’ care, using shared technology, timely data, and information about emergency department visits and hospital admissions. This collaboration will also provide services to help patients manage their chronic health conditions by enhancing relationships with their personal physicians and encouraging healthy lifestyles.

UnitedHealthcare will supplement HealthCare Partners’ own data to help support overall population health, giving the entire care team clear, actionable data about individual patients’ health needs, potential gaps in care and proactive identification of high-risk patients. 

More than 15 million people enrolled in UnitedHealthcare plans across the country have access to accountable care programs, delivered in part through more than 800 accountable care arrangements nationwide as the organization engages in deeper, more collaborative relationships with physicians and hospitals.

Care providers nationwide are showing strong interest in a shift to value-based care. UnitedHealthcare’s total payments to physicians and hospitals that are tied to value-based arrangements have tripled in the last three years to more than $52 billion. By the end of 2018, UnitedHealthcare expects that figure to reach $65 billion.
